If you do not have an attorney and wish to request a mediation or arbitration appointment, please fill out the Pro Se Appointment Request Form, including all required information. If you would like to request a specific date, If you have safety or security concerns, or if there is a protective order in your case, please make sure to let me know. Once I receive your information, I will reach out to the parties via email to confirm dates and will send the parties a mediation agreement.

To streamline the scheduling process, I recommend that you check the "Calendar" page of this site for available dates before filling out this form and indicate the free dates on my calendar that will work best for all parties.
